BAD BOY INK TATTOO CONSENT/WAIVER FORMS

I acknowledge by signing this agreement that I have been given the full opportunity to ask any and all questions which I might have about the obtaining of a tattoo and that all of my questions have been answered to my full satisfaction. I specifically acknowledge I have been advised of the facts and matters set forth below and I agree as follows:

    • I have been fully informed of the inherent risks,I fully understand that these risks, both known and un-known, can lead to injury, including but not limited to infection, scaring, difficulties in detecting melanoma and allergic reactions to tattoo pigment, latex gloves, and/or soap. Having been informed of the potential risks associated with getting a tattoo, I still wish to proceed with the tattoo application and I freely accept and assume any and all risks that may arise from the tattoo. 
    • I agree to WAVE and RELEASE to the fullest extent permitted by law each of the Artists and the Tattoo Studio from all liability whatsoever, for any and all claims or causes of action that I, my estate, heirs, executors or assigns may have for personal injury or otherwise, including any direct and/or consequential damages, which result or arise from the application of my tattoo, whether caused by negligence or fault of either the Artist or the Tattoo Studio, or otherwise.
    • I agree that Neither the Artist nor the Tattoo Studio is responsible for the meaning or spelling of the symbol or text that I have provided to them or chosen from the flash (design) sheets for a tattoo. it is the clients full responsibility to check for grammar/spelling mistakes and meaning of symbols, tattoo artists are not responsible for spelling or meaning of symbols, clients must always check before and approve before artist tattoos. 
    • If I have diabetes, epilepsy, hepatitis, hemophilia, HIV-AIDS, or any other communicable disease, heart condition or take medicine which thins the blood I have advised my tattooist. I am not pregnant or nursing. I am not under the influence of alcohol or drugs. 
    • I do not have medical or skin conditions such as but not limited to: acne, scarring (Keloid), Eczema, psoriasis, freckles, moles or sunburn in the area to be tattooed that may interfere with said tattoo. If I have any type of infection or rash anywhere on my body, I will advise my tattooist. 
    • I acknowledge it is not reasonably possible for the representatives and employees of this tattoo shop to determine whether I might have an allergic reaction to the pigments or processes used in my tattoo, and I agree to accept the risk that such a reaction is possible.  
    • I acknowledge that infection is always possible as a result of the obtaining of a tattoo, particularly in the event that I do not take proper care of my tattoo. I have received aftercare instructions and I agree to follow them while my tattoo is healing. I agree that any touch-up work needed, due to my own negligence, will be done at my own expense. 
    • I realize that variations in color and design may exist between any tattoo as selected by me and as ultimately applied to my body. I understand that if my skin color is dark, the colors will not appear as bright as they do on light skin. 
    • I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my tattoo. 
    • I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change or remove my tattoo. To my knowledge, I do not have a physical, mental or medical impairment or disability which might affect my well being as a direct or indirect result of my decision to have a tattoo. 
    • I acknowledge incase of potential infection, I am to call my family doctor not the tattoo artist, if redness or swelling persist after a few days, you have a fever, or notice a rash in or around around the tattoo. 
    • I acknowledge I am over the age of eighteen and that I have truthfully represented to my tattooist that the obtaining of a tattoo is by my choice alone. I consent to the application of the tattoo and to any actions or conduct of the representatives and employees of the tattoo shop reasonably necessary to perform the tattoo procedure. 
    • I release all rights to any photographs taken of me and the finished body art and give consent in advance to their reproduction in print or electronic form. 
    • I agree to reimburse each of the Artist and Bad Boy Ink for any attorneys’ fees and costs incurred in any legal action I bring against either the Artist or Bad Boy Ink and in which either the Artist or Bad Boy Ink is the prevailing party. 
    • I agree that the courts of Ontario Canada have personal jurisdiction and agency over me and shall have exclusive jurisdiction for the purpose of litigating any dispute arising out of or related to this agreement.
    • I acknowledge that I have been given adequate opportunity to read and understand this document, that it was not presented to me at the last minute, and I understand that I am signing a legal contract waiving certain rights to recover against the Artist and Bad Boy Ink. 
    • If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document. I hereby declare that I am of legal age (and have provided valid proof of age) and am competent to sign this Agreement or, if not, that my parent or legal guardian shall sign on my behalf, and that my parent or legal guardian is in complete understanding and concurrence with this agreement.
    • I also understand that the employees at the Tattoo Studios are operating in compliance to the Middlesex health of London, Ontario, and will give me the professional standard, in terms of sterilization, usage of disposable instruments, and artistic standard.
    • I understand that I am being tattooed in a licensed tattoo professional setting, I also understand that I am able to choose to be tattooed by either my choice of tattoo artist or I may choose to be tattooed by one of there tattoo apprentice's. 
    • I agree that the Bad Boy Ink Tattoo Studio has a NO REFUND policy on tattoos and or retail sales and I will not ask for a refund for any reason whatsoever.

I SPECIFICALLY  ACKNOWLEDGE AND I UNDERSTAND, I AM CONSENTING TO BEING TATTOOED BY MY CHOICE EITHER AN EXPERIENCED TATTOO ARTIST OR BY A TATTOO APPRENTICE WITH LESS EXPERIENCE, THE SHOP HAS ALREADY FULLY INFORMED ME OF THE RISKS AND TAKE NO RESPONSIBILITY FOR A TATTOO APPRENTICE'S  WORK, IF I CHOOSE TO BE TATTOOED BY A TATTOO APPRENTICE. I ACKNOWELEDGE AND I AM FULLY AWARE THAT BY CHOOSING A TATTOO APPRENTICE  THAT HE IS STILL LEARNING. I UNDERSTAND THAT PERFORMING TATTOOS IS NOT A TRADE THAT IS TAUGHT IN A TRADITIONAL SCHOOL SETTING, NOR ARE THERE ANY VALID COURSES A PERSON CAN TAKE FOR TATTOOING. I UNDERSTAND THAT AN APPRENTICESHIP IS A FORM OF ON THE JOB TRAINING WHERE A PERSON THE CAN LEARN THE ART OF TATTOOING AND BE TAUGHT ALONG WITH TECHNIQUES FOR AVOIDING BLOOD-BORNE PATHOGENS, HANDLING NEEDLES, AND STUDIO ETIQUETTE. I UNDERSTAND THAT APPRENTICES ARE STILL GETTING VALUABLE HANDS-ON EXPERIENCE AS THEY LEARN THE CRAFT OF BODY ART AND TATTOOING. I HAVE BEEN MADE AWARE THAT THE APPRENTICE WILL FOLLOW ALL SAFETY PROTOCOLS TO ENSURE THAT THE TATTOO SETTING IS SAFE. I UNDERSTAND THE RISKS INVOLVED AND CONSENT TO BE TATTOOED BY A TATTOO APPRENTICE.∗

BAD BOY INK TATTOO AFTERCARE INSTRUCTIONS

How To Take Care of Your New Tattoo

Follow Your Artists Instructions

Always follow the directions given to you by your tattoo artist – they will know how best to take care of your new piece and if it requires any unique attention.

Typical aftercare procedure is:

1) Leave your tattoo derm (bandaging) on for 3-4 days, unless liquid is leaking from the bandage, in which case remove as soon as possible.

2) When removing tattoo derm, it is usually easiest to do under running warm water with unscented soap while you pull the bandaging slowly downward.

3) Use clean paper towel to pat your tattoo dry.

4) Healing time will differ, but you want to wait until your tattoo feels dry (likely the case if you have kept your derm on for the recommended 3-4 days) before you begin applying unscented moisturizer.

5) Clean your tattoo twice daily (and as circumstances require), once in the morning and once at night with warm water and unscented soap, pat dry and then apply unscented moisturizer until your tattoo has fully healed (generally between 21-30 days)

Tattoo Healing Dont’s

For the first 30 days with your new tattoo, you must avoid the following: 

    • Exposing the tattoo to direct sunlight
    • Submerging your tattoo in water for long periods (no baths or swimming)
    • Exfoliating/shaving or waxing the tattooed area
    • Any unnecessary touching, especially by pets or other contaminants
    • Moisturizing with petroleum jellies or petroleum based moisturizers
    • Send a Photo of Your Healed Piece To Your Tattoo Artist

Artists love to see photos of their healed work! This also gives your artist an opportunity to determine how your piece has healed and see if you need to be booked in for a touch-up.

Long-Term Tattoo Care

The longevity of your tattoo is mostly in your hands, the better you take care of your skin, the better your piece is going to age. Moisturize often and do your best to always keep tattoos out of the sun unless you’ve applied sunscreen!

The artist has explained throughly the tattoo aftercare instructions to me and has also provided me with an electronic copy of the tattoo aftercare instructions at the time of my appointment. I have a choice to receive a printed physical copy of the tattoo aftercare instructions if I choose, by checking yes in the box I'm consenting to a electronic copy of the tattoo aftercare instructions to the email I provide. I understand that I have the right to request a paper copy instead. By checking the box below, I am waiving that right. After consenting, I may, upon written request to Bad Boy Ink Corporation, obtain a paper Physical copy of  the tattoo aftercare instructions. No fee will be charged for such copy and no special hardware or software is required to view it. By checking the box below I agree the tattoo artist has fully explained and I have been given an electronic copy of the tattoo  aftercare instructions and understand that the aftercare process is up to each individual customer to follow and any problem that arises with the tattoo after it is completed is not part of the Bad Boy Ink Tattoo Studio or Artist/ Employees responsibility.
